Here's the impact you'll make and what we'll accomplish together

As a Case Manager for the Mental Health division of TELUS Health Care Centres, you will provide excellent client care by assisting the medical and professional staff in managing client activity. You will work closely with clients to identify their needs and help connect them to the best clinician for the client. To do this, you will need to develop strong relationships with the clients and our Mental Health clinicians (including support staff, Psychologists, Social Workers, Clinical Counsellors, Dietitians, and other specialists). This will facilitate ongoing communications and cross-referrals to get the most out of our amazing Mental Health team.

Here's how

  • Work closely with clients to identify their goals and needs, and communicate with the clinical team, to help clients and family navigate through the care journey
  • Develop trust and positive rapport with all clients, providing care with kindness, empathy and compassion 
  • Provide information, support and assistance to individuals living with mental health concerns, their families and health service providers
  • Central point for clients through screening, referrals and provide high quality comprehensive information on private and community mental health resources and services
  • Facilitates informed consent process 
  • Administer and process client information and requisitions
  • They will help to guide referrals for tertiary or crisis care where needed
  • Schedule a wide array of appointments, programs and services 
  • Facilitate, track and document completion of prerequisites tests and applicable diagnostic procedures
  • Monitor and facilitate patient compliance and readiness for treatment
  • Answer and triage telephone, office email, and messages from multidisciplinary team

You're the missing piece of the puzzle

Experience/Knowledge:

  • Minimum Bachelor-level education or higher
  • Minimum 5 years of work experience in Mental Health-related field
  • Preference given for nursing experience
  • Comfortable with online systems and platforms such as Collaborative Health Record (our EMR), Microsoft Office, and Google Suite
  • Medical Office Assistant experience an asset
  • Knowledge of medical terminology preferred

Skills and Abilities:

  • Good knowledge of Microsoft Office 
  • Exceptional organizational and customer service skills
  • A team player capable of interacting professionally with staff and Health Care provides with a positive attitude
  • Ability to handle and communicate effectively with challenging clients

Work Conditions/Physical Requirements:

  • You will be required to work in a multi-disciplinary clinical setting. Typically you would work office hours however; some flexibility may be required to accommodate service requirements for the operation
  • You will be working in an office environment, which may require extended periods of time completing computer related work. You will work an eight hour day with an additional minimum of half hour for lunch. You may be required to travel for meetings and centre support
